- Complete and submit the tubal ligation patient information form. You must provide your current height and weight so our surgeons can determine if the procedure can be performed successfully. Falsifying information on these forms may result in the cancellation of your surgery and forfeiture of fees. Please note that you will be weighed on the day of your surgery.
- Complete the Medical Records Release form so we may obtain your operative notes from the facility or doctor who performed your tubal ligation. Ensure that you provide the correct contact details. If you cannot provide details to obtain your medical records regarding your tubal ligation, please complete a Medical Records Waiver Form instead. If you have your medical records, please use THIS FORM to upload them securely.
- After reviewing your operative and pathology reports and patient information form, our team will contact you to confirm your approval for surgery. If surgery is not deemed the most suitable option based on your medical history and provided information, we will recommend an alternative course of action tailored to your needs.
