4. Create your user name and password
5. Visit the “Appointments” tab
6. Select “Request Appointment”
7. Submit your request and a scheduler will reach out to you to schedule your appointment

2. Select request an appointment
3. Select appointment type, preferred time, and provider
4. Submit your request and we will reach out to you to finalize and confirm your appointment
